  • Full credit/debit card numbers

5. HIPAA Clarification

The Health Insurance Portability and Accountability Act (HIPAA) applies to "covered entities" such as healthcare providers, health plans, and healthcare clearinghouses, as well as their business associates who handle Protected Health Information (PHI).
